RIGHTS OF CLIENT, KERRY KOTT, AND THOSE ACTING ON THE BEHALF OF KERRY KOTT TO USE OF VIDEO, AUDIO, PHOTOGRAPHIC, DIGITAL, ELECTRONIC OR ANY OTHER MEDIUM BY KERRY KOTT.

I understand that I am participating in the Nude Nature Project with Kerry Kott.

I understand by request of this experience that I am agreeing to be photographed completely nude outside in nature at an agreed upon location by me and Kerry Kott.

I give Kerry Kott permission to record my likeness and/or voice on a video, audio, photographic, digital, electronic or any other medium.

I understand this photoshoot is provided for me in exchange for use of photographs. I understand that no royalty, fee, or other compensation shall become payable to me for this release at any time for any reason.

I authorize Kerry Kott and those acting on her behalf wholly or in part use of all video, audio, photographic, digital, electronic or any other medium taken of me during this session for use to reproduce, exhibit, and/or distribute in any publication; commercial or otherwise for promotional, advertising, educational, and/or other lawful purposes; including website, social media, internet, print, etc. I understand that no royalty, fee, or other compensation shall become payable to me for this release. I release and waive any claims or rights of compensation or ownership regarding such uses and understand that all such recordings shall remain the property of Kerry Kott.

I understand the video, audio, photographic, digital, electronic or any other medium may be used to represent an imaginary person and any wording associated with it will not be attributed to me unless my name is specifically mentioned. Provided my name is not mentioned in connection with any other statement or wording which may be attributed to me personally, unless I give clear permission for use of my name noted in this document, I undertake not to Prosecute or to institute proceedings, claims or demands against either Kerry Kott or her agents in respect of any usage of all video, audio, photographic, digital, electronic or any other medium captured of me.

I understand that all video, audio, photographic, digital, electronic or any other medium are 100% for personal use only and I am not authorized to sell or use any video, audio, photographic, digital, electronic or any other medium captured by Kerry Kott for commercial use unless authorized by Kerry Kott.

I understand that Kerry Kott and I will review and delete any necessary video, audio, photographic, digital, electronic or any other medium captured during our photoshoot together. I understand the ultimate decision on which to save and share are under the sole discretion of Kerry Kott. I understand that the remaining video, audio, photographic, digital, electronic or any other medium captured will be sent to me digitally via Lightroom within seven days of the photoshoot.

I understand that I am not authorized to manipulate and edit any video, audio, photographic, digital, electronic or any other medium by Kerry Kott without the permission of Kerry Kott.

I understand that any physical prints are my sole responsibility in acquiring. I understand that Kerry Kott has made available third party resources for printing, to which may be affiliate accounts in which Kerry Kott may receive monetary compensation for my use of their service. I take full responsibility for use of any third party companies and release Kerry Kott from any liability in connection with third parties.

I understand that sharing any video, audio, photographic, digital, electronic or any other medium captured by Kerry Kott of me with any third parties is my personal choice and I take full responsibility of any and all outcomes of my sharing and release Kerry Kott from any liability in connection with third parties.

I understand that Kerry Kott holds sole copyrights as the photographer and if I choose to share any and/or all video, audio, photographic, digital, electronic or any other medium captured during our session together that I will correctly credit Kerry Kott in writing; by use of name, “Kerry Kott”, website “kerrykott.com”, tagging on social media and/or any other appropriate identifying means.

I understand I may wish to share a testimonial of my experience, via video, photographic and/or in writing and Kerry Kott will provide me with an authorization form in which I choose how and what information about me I’d like to share.

I hereby release Kerry Kott and those acting on her behalf from all claims and liability relating to video, audio, photographic, digital, electronic or any other medium captured of me.

I hereby assign full copyright of these photographs to the above-mentioned photographer (and the related representatives and assigns) together with the right of reproduction either wholly or in part.

I understand that with my authorization the video, audio, photographic, digital, electronic or any other medium may never be revoked per this agreement.

I understand that I may wish to revoke consent of use to Kerry Kott to which I will request in writing. If revoking authorization of use, I understand it will not affect any actions taken before the receipt of this written notification.

I understand all video, audio, photographic, digital, electronic or any other medium will be stored in a secure location.

I have read this PHOTOGRAPHY RELEASE FORM carefully and fully understand its meanings and implications.

I certify that I am 18 years of age or older on the date of signing.
